To view the Insurance Information screen, select Insurance Information from the Health Info tab on the customer record. The Insurance Information screen will display, as shown below. From this screen, you can edit a customer's insurance information. Editing Insurance Information The fields on the Insurance Information screen are free text fields. This information can also be updated by the customer from their web profile or during online registration. To add or edit insurance information: From the Insurance Information screen, click the pencil icon in the upper right corner of the screen. The Insurance Information screen opens for edit, as shown below. From the Insurance Information section, perform the following: By default, the Carries Health Insurance? option is set to No, making the fields in this section unavailable to edit. If you wish to add insurance information, select Yes. Enter the name of the Health Insurance Carrier, if necessary. Enter the phone number of the carrier in the Health Insurance Carrier Phone field, if necessary. Enter the Name of Insured, if necessary. Enter the Group Number, if necessary. Enter the Policy Number, if necessary. Enter the Relationship to Participant, if necessary. From the Personal Information section, perform the following: Enter the Physician Name, if necessary. Enter the Physician Phone Number, if necessary. Enter the Physician Address, if necessary. Enter the Zip Code of the physician, if necessary. Enter the City of the physician, if necessary. Enter the State of the physician, if necessary. Enter the Dentist Name, if necessary. Enter the Dentist Phone Number, if necessary. Enter the Dentist Address, if necessary. Enter the Zip Code of the dentist, if necessary. Enter the City of the dentist, if necessary. Enter the State of the dentist, if necessary. Click Save. Questions?
