To view the Health Forms screen, select Health Forms from the Health Info tab on the customer record. The Health Forms screen displays, as shown below. From this screen, you can edit a customer's health form information. Editing Health Forms Information The fields on the Health Forms screen are free text fields. This information can also be updated by the customer from their web profile or during online registration. To add or edit health form information: From the Health Forms screen, click the pencil icon in the upper right corner of the screen. The Health Forms screen opens for edit, as shown below. Select the date of the Signed Participant Waiver from the date chooser, if necessary. Select the waiver branch from the Please Select Waiver Branch drop-down. Select the date of the Health History from the date chooser, if necessary. Select the health history branch from the Please Select History Branch drop-down, if necessary. Select the date of the Authorization for Treatment from the date chooser, if necessary. Select the authorization branch from the Please Select Auth Branch drop-down. Select the date of the Activity Form from the date chooser, if necessary. Select the authorization activity branch from the Please Select Auth Activity Branch drop-down. Click Save. Questions?
